Registration Fees :
1000/- (Rupees One Thousand Only) vide Demand Draft (DD) drawn in favour of Silver Dream Entertainment, Allahabad Bank, Southern Avenue Branch, Kolkata – 700029.Please send the completely filled application form, photographs alongwith the demand draft to Silver Dream Entertainment, 1/16, Sa hid Nagar, Dhakuria, Kolkata – 700031 via courier or speed post. The form can also be manually submitted tothe aforementioned address Mon-Sat between 10 AM and 6 PM.

Terms & Conditions :
1. The applicant must agree to abide by all Rules, as changed from time to time by the Organizers
2. The applicant should be between 18 – 25 years of age as on 31st December 2016.
3. The applicant uses the prefix ‘Miss.’(or equivalent) before her name

Declaration :
I have read and fully understand and agree to abide by the Rules and Regulations for “Miss India Beauty 2016” contest, which are incorporated at the end of this form for Reference. I further agree to strictly honour the terms and conditions of this agreement and understand that any violation of the same will call for immediate disqualification.

I hereby release and discharge “Miss India Beauty 2016” Organizers, its successors and persons, officers, authorized Representatives, individuals connected with it from any responsibility, liability or accountability of any kind whatsoever in connection with this pageant. I hereby expressly and unconditionally discharge “Miss India Beauty 2016”.

The participant agrees to indemnify and compensate “Miss India Beauty 2016” organizers for any claim, loss, expenses damage etc. that may be suffered by them as a consequence of any acts of commission or omission by the participant. I also certify that the information presented in this form is correct and true to the best of my knowledge and nothing has been concealed. I also understand that the organizers Reserve all rights pertaining to this pageant and their decision shall be final and binding on me.
